Financial Information

Our doctor(s) and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy below.

Our offices are fee-for-service and all payment arrangements are between the doctor and patient. Unless otherwise noted, insurance benefits are considered a form of reimbursement to the patient and not a means of payment.  Therefore, all treatment fees are the responsibility of the patient.  We will assist you in filing your insurance for your reimbursement.  

Examinations and Hygiene Cleaning Visits

Our policy is payment in full due at the time of service. For your convenience we accept cash, checks, money orders, and credit card payment (MasterCard, Visa, and Discover).  We can assist you in filing your insurance for your reimbursement. 

Emergency/Single-Visit Payments

For single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders, and credit card payment (MasterCard, Visa, and Discover).  We can assist you in filing your insurance for your reimbursement. 

Multiple Visit Payment Options

All payment arrangements are between the doctor and patient. Unless otherwise noted, insurance benefits are considered a form of reimbursement to the patient and not a means of payment.  Therefore, all treatment fees are the responsibility of the patient.  We will assist you in filing your insurance for your reimbursement.  

Option 1: Full Payment at Time of Service – 5 percent fee reduction for full payment before treatment is rendered (cash or check) and 3 percent fee reduction if paid in full with a credit card (MasterCard, Visa, and Discover).

Selection of this option gives you the maximum savings on the cost of your treatment plan because the billing expenses we save are passed on to you in the form of a 3 – 5% discount. Your Treatment Coordinator will provide you with specific details.

Option 2: Account Payment Plan – Half of the treatment fee is due at the time of service and the remaining balance is divided into two equal payments at 30 and 60 days.

For individuals desiring a payment plan, the Account Payment Plan may be the best choice. This plan allows for payments to be paid out over a period of time.  The balance remaining after the day of treatment is split into two equal payments:  one payment due one month after treatment, and the second and final payment is due two months after treatment.  

Option 3: Outside Financing – For those who would prefer an extended payment plan.

Our office partners with Care Credit to offer patients extended payment plans that are interest free.  Please ask one of our team members if you are interested in discussing Care Credit to help with your treatment investment. 

**For your convenience, payments may be made with cash, personal check, Discover, Visa, American Express or MasterCard.  We will assist you in filing your insurance for your reimbursement.
